Purpose

Please use the search options below to find providers in MINES PPO network. If you are looking for providers in the MINES EAP network, you must call MINES at 800-873-7138.

Instructions

  1. You can search MINES network facilities and outpatient providers by their name (must be their legal name) or by your zip code. This search will only pull up providers within 20 miles.
  1. You can search by provider’s specialties by using “Specialty”, “Other”, or “Settings”.
  1. If you would like to locate a provider that has multiple specialties, please note the following:
  1. On a computer: Hold down “CTRL” while you click on all checkboxes you want to select.
  1. On a mobile device: Click on all checkboxes you want to select.

Provider Availability

Please note that we do our best to ensure that the MINES facilities and providers listed below are actively taking new clients. MINES may have additional providers in our network that are not listed as they are only seeing current clients or work with us on a single case basis. This list is updated at least every 30 days. However, please note that we do not guarantee all providers listed below are taking new clients. When you contact them to schedule, please let them know that you are using the MINES PPO network for your behavioral health needs and confirm they have openings.

Provider Network Status

It is the provider and client’s responsibility to verify network status and benefit details. If you choose to see a provider that decides to drop out of our network, we do ask our providers to inform you of their decision and follow up with you about continuing with them through self-pay or concluding services. If you need additional referrals to providers in your network, you can use this search tool or contact us at 800-873-7138.

CAA compliance: Please note that per the CAA ruling that you, as the client, can elect to see the provider up to 90 days from their termination date at in-network rates (to help you with your transition). Please discuss this with your provider should they inform you of their termination with us and fill out the required form.
